To begin the process to retire a policy, please complete the request form below, which will be sent to the policy library coordinator upon submission of the form. The policy library coordinator may contact you for any clarifications. You will receive confirmation that the policy has been retired.
If you need to submit multiple policy retirement requests at the same time, the policy library coordinator can work with you to streamline the submission process. To discuss this option or any questions you have about the policy retirement process, please email firstname.lastname@example.org.
